(6) As of the effective date of termination, a certified marriage and family therapist is no longer recognized as a licensed provider under CHAMPUS. After termination, the certified marriage and family therapist can only be rehired by entering into a new participation contract as a certified marriage and family therapist as an authorized CHAMPUS additional physician. (i) Either 200 hours of supervision authorized in the practice of marriage and family guidance, which must normally be completed over a period of 2 to 3 years, of which at least 100 hours must be under individual supervision. Such monitoring should preferably be carried out with more than one supervisor and should include an ongoing monitoring process involving at least three cases; and (v) terminate the approval of a case if it is found that there has been a deviation from the facts on which the authorization was originally based. . . .
